If participating at store: I understand that it is my responsibility to obtain my clue and fabric packet from the store and that I will not be mailed a fabric packet and clue and agree to the class policies of Kate's Quilting Block.  I understand that this is a one time registration fee of $25.00 and a recurring charge of $15.00 a month (2-12).  

If participating via mail: I understand that this is a one time registration fee of $25.00 and a recurring charge of $15.00 a month (2-12) plus shipping & handling. Shipping & handling charges are $4.95 per month for each clue and fabric packet. Participants will be registered for one year. Packages will be shipped around the 1st of each month. If you choose to drop out in the middle of the year, please email us prior to the 15th of the month before the next shipment.
